Getting Started
Prior to your visit, we invite you to download, print and complete some of our new patient forms. Please bring these forms to your initial visit. These forms will need to be completed prior to any treatment. Please also bring your physical therapy referral from your doctor along with your insurance card and drivers license. If your injury is through a workers compensation case or an auto claim please have the insurance contact information and claim number.

Why we don’t accept your insurance
1) The most common reason we may not accept your insurance is that their rates are to low to provide a quality service. Low rates mean that a therapist has to see many more patients in a day, which lowers the quality of care. Some insurances have not raised their rates in over 10 years
2) Your insurance may have other policies that have made them very difficult to work with along with subpar reimbursement rates. Examples are excessive authorization procedures, poor communication with getting benefits or authorizations and administrative red tape. Insurances have drastically worsened their reimbursements and restrictive policies creating a difficult environment and becoming prohibitive to work with.
